Questionnaire landing page

I need to create a landing page for my business.

The landing page needs to start with a short questionnaire with score interpretation. There are 5 questions and I would like each question to be on separate page as opposed to in a list. The quiz will give the user a result which is scored from each answer.

After the questionnaire there will be a simple one page landing page. The call to action will be to book an appointment therefore a booking system will need to be integrated.

I will host the page through my own hosting on Hostinger, therefore I expect WordPress would be the best platform.

The developer will need to do SEO optimisation and connect to google analytics.

I have company logo and content available for an immediate start.
