Convert two surveys on word document to online surveys

I have two surveys, which are currently on Word Documents. I've attached both of them. I need to be able to distribute these via online surveys, using a tool like Typeform, Surveymonkey or Smartsurvey etc. I dont mind which of the tools is used and will create an account for whichever is recommended. But I need someone to take the word documents and use the questions to create the online surveys. Not sure how long this will take so please suggest how long it would take in your proposal.
