Add newsletter sign up button to website

I would like a button added to six pages of my website for visitors to sign up to my monthly email newsletter.

This button is to take the visitor to an embedded Mail Chimp form to add them to the subscriber list (see copy attached).

I also want an automated email response set up from Mail Chimp confirming their subscription and with an offer (see copy attached).

My website is Word Press, hosted by Fasthosts.

My understanding is that it should be only about an hour's work.

This is not urgent project, but I would like it completed within a couple of weeks.
