Logo creation - design/concept given, just put it all together!

I need to refresh a logo. Tasks to be done (see attachments):

1) CREATE A LOGO WHICH WILL CONSIST OF THE WAVE, VERTICAL LINE AND BRAND NAME (see Model.png)
- wave will be provided as ai/eps/psd file (I'm 99% sure it can be re-used, there is a tiny chance it'll have to be replicated based on jpg/png files)
- "ultimate" word to be replaced with another brand name (7 characters, single line)
- brand name design should be similar to "ultimate" (shadow, 3D etc.) but far less shiny, almost white with minimal 3D effect - TBC when I "see it"
- similar effect to be added to logo (wave)
- dimensions (see Dimensions.png) should be (in pixels): A=35, B=45, C=29
- there will be a few iterations to ensure the final version is as expected

2) ONCE STEP 1) HAS BEEN COMPLETED, CREATE A FEW SIZE & COLOUR VARIATIONS
- sizes (in pixels, high quality jpg format):
A=30, B=39, C=25
A=35, B=45, C=29 (already created above)
A=70, B=91, C=58
A=100, B=129, C=82
A=150, B=194, C=124
A=200, B=259, C=165
A=300, B=388, C=247
(supposedly only scaling, ensuring one of the dimensions is as required)

- colour variations with the above dimensions:
a) black colour, no shadow, no 3D effect ("flat") - 4 selected sizes I'll select from the list above, transparent background
b) white colour, no shadow, no 3D effect ("flat") - 4 selected sizes I'll select from the list above, transparent background


Requirements in regards to the freelancer and job:
- I want see your portfolio proving you are able to complete this job
- Completion time: 6-7 calendar days maximum from proposal acceptance day, where first 3-4 days will be spend on iterations to complete step 1)
- Daily, ideally, twice-a-day checkpoints/iteration rounds for completing step 1) to ensure it's done quickly and efficiently
- Skype IM comms preferred
- Deliverables: high quality jpg files as listed above + single PSD file (or XCF) and two PSDs/XCFs for monochrome versions
