Design for DVD sleeve, label and image for website

We are producing a new DVD product which will contain IT training videos for a specific niche programming environment. We need the following to be designed:

- the DVD label
- the DVD box sleeve
- an image to show the DVD product on a sales page (eg on Amazon.com)

The bidder should therefore be able to produce and provide 3 separate images, as photoshop files. The images must use the existing company logo and colour scheme, as per our website, which is available on request - the winning bidder will be responsible for coming up with an appropriate layout on the disk. Any ability to provide advice on other text to go on the sleeve or disk label would be a bonus.

The DVD has already been authored and so no input is required into the DVD itself, only its\' packaging.
