    Hello Lee, it's Shahab. I've been reviewing your form and noticed you require a space for a statement box to accept the terms and conditions. Unfortunately, there appears to be limited space on your current PDF form to include a new statement box on the same page. Would it be suitable for you if I incorporated a single sentence for the acceptance of terms and conditions alongside a checkbox?
