Experience Level: Intermediate

After several failed attempts by two others at this job, I would very much appreciate some genuine help on this project and a fast turnaround.

I have attached a version which is 80% complete but still not usable and a word document explaining what I need amended.

    I need an update to the PDF as described as long as the free txt boxes work and the tick boes / drop down boxes etc then that's is all.
