Forms Designed with Fillable PDF (Adobe Esign & Print)

I am looking for a designer who has experience in designing forms that I can use as a fillable PDF for Adobe E-sign (or similar platform) and for printing for clients to sign in person.

There are 2 separate documents, which are contracts with terms and condition (one for business clients and other for consumer clients). They are very similar but just have slight differences. Both 19 pages long in total but only 5 pages of forms on each with the rest terms and conditions:

• Page 1 - Contract Details Sheet (Terms and conditions will come right after but will be in the same format (2 columns but have good spacing).
• Pages 10 - 13 - Schedule 1 Order Form
• Page 14 - Schedule 2 - The Services - Just looking for a nice design to this - not a form.
• The other wording (Schedule 3 Supplemental Terms) needs to come after, it can remain in one column or two depending on design.

Design Brief:
• New front page design with just with our branding logo and title of the document. As we have two separate documents (that are similar) we would just have a slightly different design to each one so its clear.
• Forms can be opened as it looks too complicated, it needs to be clear as we will be asking clients to complete this online themselves when we send this to them via adobe e-sign.
• The text can be opened slightly. I don’t want to make the documents too long with many extra pages, but it should look presentable.
• Colours should be in line with branding.
• Design needs to be suitable for completing online (via adobe e-sign) and printing to be completed by hand.

These are two separate documents but have a common structure.

Important: The text and numbering of the document must NOT be changed. I just want these documents put together well so it looks presentable.

I am looking for this project to be completed within 3-5 working days maximum.
