Design Double Sided A5 leaflet (will be folded)

I am looking for someone to create a double sided A5 (folded) leaflet which will be placed inside a product that customers buy online.

Requirements:
- 2 sided A5 (folded)
- Text and some branding images will be supplied
- The leaflet is a tie up between two companies, giving added value to the customer who has purchased a product and then gets added value from the offer on our leaflet enclosed in the product they have ordered.
- The leaflet will explain what we do and can offer, it is an after purchase offer (text will be provided) and will also list the benefits of our unique service.
- Needs to be a creative design, as opposed to just text on paper, needs to be thought provoking
- Timescale – 2 weeks
