Design a Wedding Makeup Artist booking form .

I have used an old existing booking form and I wish to utilize the same form , but designed a little more professionally.
I would also like the form to be developed so resides on the desktop in electronic format, with the option to save and print individual populated or blank hard copies also.
From the booking form I would like to utilise some of the data from the form to build a database of products used, prices of weddings and more.
I'm looking for inspiration and inparticular good professional ideas.
