Create an A5 leaflet/Sandwich Board Artwork from information provided

Design details: We would like the information contained in the attachment creating into both an A5 leaflet and also to be printed onto a sandwich board.
We want to be able to send both designs straight to the printers for them to reproduce with no design needed.

Extra notes:

We dont need the sandwich board making, just the artwork for it, as I already have a local printers ready to print. There is no printing involved in this job, just the artwork, which we have the basic design for, we just need it making more pleasing on the eye.
