Create a great looking certificate in PDF

I’m looking for someone to design a professional training certificate PDF where I will handwrite the persons name and date etc.

Here’s is the wording I need on the certificate

“This is to certify that .................................has completed a 1-1 workshop with Jayne Todman Nail Design


Date........... Signature...................”


The design should incorporate my logo (attached) as seamlessly as possible.
I have also attached a certificate that represents the style I am looking for, although I don’t like the box the logo is in.
The certificate should be A4 portrait.
