Convert Application Form form PDF to editable PDF

I have an application form in PDF which people can download from the website and then print out fill it and scan it and then send a an email copy to my email.

I would like to convert the same design in Fillable form and save the data directly on PDF rather than sending the data to Email. When fill the form on PDF then save it there and then send the PDF as attachment. which I can print out just as the printed format of the form.
