Add interactivity to PDFs

I have 5 PDFs that need interactive text boxes created. An example of one of them is attached. I will forward on the whole set to the awarded freelancer.

PDF1 - 2 pages
PDF2 - 4 pages
PDF3 - 4 pages
PDF5 - 4 pages

Everything is only text boxes, no complicated elements like check boxes or radials etc.

Requirements:
- DO NOT LOCK THE PDF from being further edited
- Neat boxes please
- Type inside the boxes to be consistently formatted to Lato 11pt
- Delivery by Sunday 9pm Brisbane time (first drafts earlier if possible so that i can check them over).
