2 x Interactive PDF forms (re-build and changes)

I am attaching two interactive PDF forms to this job sheet, which need re-building in order to make the following amends (I don't have access to the original InDesign files)

I think it is an opportunity to update the look of the forms too, to make them look fresher and cleaner. But, the Fostering in Oxfordshire and Oxfordshire County Council logos must remain.

The changes to the two forms are as follows:

Application Form:
• Last page: ‘Signature/s of Children 18 years of age and older’ – should include all household members

Reference form:
• P. 3 Employer references – to include voluntary work
• Add box for previous employment or voluntary work with children or vulnerable adults – use same headings as employer reference (apart from NI no)
• Have you ever served in the Armed Forces or been married to someone in the Armed Forces? Y/N
• Have you ever been a registered child minder Y/N
If yes please give URN and date……………………….
• P.5 school references - to include Nursery/pre-school
