A5 Leaflet Design

I am looking for a 2 sided A5 flyer which will combine the information which I have attached in the pdf. I am trying to work with this company (Jimco) but as a separate arm and need to intergrate as much as possible to get maximum output from the flyer. We will discuss in further details what additions and subtractions can be made ie images, my own logo (Sciteck) text etc.
