A4 Loyalty Scheme voucher

We are looking for someone to use our exisiting branding and colours/design/theme to come up with an attractive Loyalty certificate in A4 that we can print on to card and edit ourselves to add individual names and amounts. This project is ideally and ASAP job and so would need a pretty quick turnaound. We have a mock up of the certificate but would just like it to be more appealing.
