3D CAD Model self assemble enclosure

Design details: Hello,
I am the product engineer at Nortech Management currently tasked with the design of a quick assembly enclosure to house one of our products called a “NX42”.
For the time being I am calling this project the “NX42 Popup enclosure.

I need a 3D CAD designer to create the enclosure design. I will providing a sketch assembled using ‘google sketch up (free version)’, CAD designs from a previous project very similar to this and the NX42 PCB for the designer to physically check the measurements.
The design will incorporate clips and ‘Living Hinges’ which are detailed on the website of the manufacturer we will be using: www.protomold.co.uk

At this stage I am after a list of suitable CAD designers and quotes.
I am expecting the job to consist of:
Detailed consolation of the project, after I will provide all available information and materials relevant.
Draft design supplied at the very least in PDF and in flat and folded up views for me to review
Following design (hopefully final) for review supplied in various formats for me (without CAD software) and the manufacture.
Ideas for the visual style: for review I will need a PDF of the model infolded in 2D and assembled in 3D. Dimensions must be included
Things to avoid: I must have a 3D model
