I need a Book Layout Designed

I need someone to add professional design, copyright information and contents list to a new version of my book: 'Great or Poor'

So it looks well laid out, clear, interesting and easy to read, with Chapter Headings and paragraph headings in appropriate places and with consistent design.

This isn't a complicated job, but one that I need someone with good attention to detail and who will put the time in to make it look really good.
