Formatting inside manuscript

I need a designer who can use Word and format inside of manuscript book for CreateSpace. The work has to be done as soon as possible. I need the pages to be numbered, index page, spaced 1 1/2 spaces and enough room for the "bleeding" when the cover is made. There will be pictures, black and white, to be added to the back part of the book. And, it must be format ready to drop into CreateSpace.
