Design Cover AND Format Journal/Book

NDA will need to be accepted for this work.

I need a designer who can:
1. Create an exciting cover page for my Journal/Book.
2. Format the Journal/Book.
3. Create exciting Lined blank pages which will need to be included within the Journal at three junctures.
4. The Journal/Book should be formatted for upload to Kindle for the paperback version and one in color and the second (same Journal) in black and white.
5. The same Journal/Book should be ready to go to print should I choose to give this to a printer locally.
6. Whilst I have tried to create a cover page, I do need a professional to give their opinion as I do not want any copyright issues!
7. I will provide 7 illustrations which will also need to be included.
8. PLEASE do NOT accept this project if you are further outsourcing the work as I will not be increasing an agreed price.
8. This will be an ongoing project so I am looking for a reliable person.
