Brochure design required - approx 16 pages

Hi,

I am looking for a brochure design for my company. I am currently working on my logo but this should be with the chosen designer over the period of designer.

The concept is a A5 size brochure type document that can actually be filled in, a lot of the design will be around branding of the booklet.

I am open to offers.

16 pages maximum with 4 pages being duplicates of designed pages.

This booklet will be filled in and signed at different points through processes.

I am after someone with experience, good English and can fully help as although the image is there this will be for someone who is creative.

This does need to look professional as will be used in a professional industry in many elements.
