Book cover and contents layout

I need someone to design a front and back cover for my A5 sized book - using the pics I'll provide. I also need the contents transferred from Word to pdf format. I have a foreword, table of contents (which needs pagination)and all the chapters which I've proof read. The instructions for the layout can be found on digitalprinting.co.uk who will print the finished item. Essentially, the Word contents need to be put in one pdf and the cover needs to be a two page pdf.
