Contractor / Ltd company advice needed

I've been offered a 12 month contract working as a Project / Movements planner. I've never done contract work before. I believe it's outside IR35. Agency said I'd need to set up a Ltd company.
Salary is between £27-30 per hour which I still need to negotiate.

I need advise on the following:

Setting up a Ltd company - preferably do it for me!
What tax / NI contributions do I need to pay and when.
Can I claim costs towards my mileage? It'll be about 90 miles total per day.
What else can I claim for? I'll be working on site, not from home. Food / drink?
Anything else I need to know about becoming a Contractor for a large company?
