Create a salary increase calculator based on performance criteria

We don't have annual pay increases each year, so when an employee asks for a salary review we go back to the Line Manager to obtain feedback, and they are scored on specific criteria. I have attached a screen-shot.

What we need now is to plug this criterion into a spreadsheet which comes back with a recommended % of what the increase should be up to 5%.

Excel and I don't mix! So someone that has intermediate experience with excel and experience in HR specifically with benchmarking salary.
