Editable PDF Creation

Description of support work: We have two PDF forms that we use to register our customers which requires a potential customer to print them off, fill them in and send them back via fax or email. We would therefore like potential customers to be able to complete the form on screen, filling in their details in the revelant field, add an electronic signature and email it back. If it is possible to attach documents to the form that would be great too. I have attached both the forms.
