Copytype a 88 pages long English PDF document into MS word (don't use OCR technology)

Hi,
I require to copy type a complex 88 pages long PDF document into word. Including a table of contents / and financial tables.
All necessary formatting, as well as proofreading, needs to be done before delivery. Work needs to be accurate.
!!DO NOT USE OCR TECHNOLOGY!! - as we need to process this document further and this would result in formatting issues not to say trouble.
The document in question is attached.

Please, quote a fixed price as well as a realistic turnaround time.
I am not willing to renegotiate after awarding the job.
