I need help creating a data entry form using Excel VBA

I've been trying to create a data entry form within Excel using VBA. Although my front-end Excel skills are better than most, I am really struggling with VBA.

What I'm trying to do is fairly simple:

1. Populate a few fields with data.
2. Click on a button to move the entered data into a table on a separate worksheet, or click on another button to remove the entered data from the form.

Please see the attached image for an idea of what I've created so far.

Should be simple for a reasonably smart guy, right? WRONG. I've been tearing my hair out with this issue, and I would like some assistance with this problem and may one or two other issues. I've written some of the code already and I just need someone to identify the reason why it's not working and then fix it. I'm hoping this will be easy money for the right VBA expert.
