I need a word document converted into a spreadsheet

I have a word document which is an index separated by spaces only and I want to convert this into an excel document. This is for our local history society. It comprises of a list of subjects and alongside it is the journal number and the page number the article appears in. What we want to end up with is a spreadsheet with two columns, one with the subject, one with the journal number and page number (sometimes there are several of these so we would need these separated by a comma.
Should be an easy job for someone who knows how to do this. I have included the full word list.
