Formatting of MS WORD document file

I require the formatting of an MS word document (attached) which would then serve l as a WORD template.

The document content and particularly the tables contained within the document MUST be fully and easily editable so information can be added as required. Document to be professionally laid out using whatever FONT type/style suits as long as it is consistently used throughout the document and tables. Note the content is an incomplete exemplar.

I need this document by NO LATER than 0900 UK GMT. I have indicated a top cert rate so my CLEAR expectation is this will be completed accurately and meet all stated requirements.
