Fillable pre printed form

I have a double sided application form which will be used by customers to enter their details onto the pre printed application, in their home. I will be having these pre printed by a professional printers.
I also receive applications by telephone and I want to be able to fill the same form by loading them into my printer, which prints to both sides, filling out the questions on my computer and then printing the answers onto the pre printed forms.
A word template with the questions alongside fillable answer fields and drop down boxes which are linked to the data fields on a template of the application forms.
When complete I press a 'print' button and the completed template prints only the filled data fields onto the pre printed application forms loaded into the printer.
I simply want a desktop icon that i click, fill out the questions and then click print.
The 2 sides of the new application form are attached........ also attached is the current template we use which prints the application form onto blank paper. This should give you a good idea of what I want.
