Create a form on excel

I have a receptionist that takes telephone calls. I want to create a form within an excel page (I don't want the excel form feature which you have to open) for her to input data in simply and easily. This data should then enter a table so I can search and edit the table.

Ideally the form should be embeded within an excel page (page 1) and the data table on page 2.

The information on the form is to be
Date of enquiry (ideally automatically populate)
Name
Contact email
Contact Mobile
Source
Enquiry Regarding (drop down options - )
background about enquiry
Next follow-up plan (notes text box)
"Appointment Booked? (yes)" (if yes highlight as blue)
