Create a fillable PDF Form to send to Clients

I have some .DOC documents that I would like converted to fillable PDF forms that I can sent to clients to complete and return.

Please see below the number of forms and pages per form:

Form 1 x 5 pages
Form 2 x 4 pages
Form 3 x 2 pages
Form 4 x 2 pages
Form 5 x 11 pages - we would need to edit this form (if possible) to change fees etc.

The forms will require mainly text boxes, tick boxes and an electronic signature.
