I need a warning letter written addressed to Dr. X to basically summarize the below incident:-

The patient came in to the clinic complaining of pain. Dr A saw her as there was no GP available and prescribed antibiotics and analgesics.

The patient started treatment with Dr. M, Dr. M initiated endo treatment, then for some reason the patient did not feel comfortable asked the reception to change dentist. Dr. X saw her did her assessment, saw that the tooth is not restorable and extracted the tooth.

after reviewing the x-rays, notes and talking to the doctors:

Dr. M:
1- Possible un-intentional perforation due to overcutting/ possible high point leading to fracture/ possible lack of properly diagnosing the tooth from the beginning that it had a crack. At least one of them is true in this case.
2- insufficient information in the notes.

Dr. X:
1- its is confirmed that she told the patient "the other dentist perforated the tooth" without taking into consideration all factors endangers the other doctor and the clinic.
2- however clinically, Dr. X did the sound clinical decision, as there is no doubt on the xray and the intra oral image showing a deep crack that the tooth is not savable .

I recommend the following

1. Dr. X, a warning letter regarding telling the patient the other doctor made a "big mistake".

reasserting that we are one team, and that we should all work together for the highest quality patient care while maintaining a healthy friendly environment amongst the team members.

The warning should include the above details as proven allegations + recommendations on how to improve + consequences should a similar incident reoccurs

The warning should be written in a professional and legal manner
