Description
The PDF report (which needs creating as part of this project) can be 90-95% pre-created through this project, we just need the ability to auto populate the personalised results such as their details and their recommendations.
This project is good to go ahead in the next 30 to 60 days, I just need individuals to give me their price and let me know what else they would need from me in order to go ahead with this project, as well as time frames.
Budget is based on PPH recomendation - Simply give me your realistic price when offering

Hey Jack, before we finalise the build route, should the first version simply generate the personalised PDF from each manager’s input without storing client data, or would you prefer a light internal record of each completed health check so it can be reviewed or matched to HubSpot later?

A few questions from my side:
1. Do you already have a sample PDF report/template, or should we design the report from scratch?
2. Would you like assessment records stored in the system for future reference, or should the system simply generate and download PDF reports?
3. Will there be different user roles like Admin and Field Managers, or only Field Managers using the platform?

Before I provide a fixed price and implementation plan, I'd like to understand a few details:
• Approximately how many health assessments do you expect to complete per week or month?
• Will the field managers use tablets, phones, laptops, or a mixture of devices?
• Do you want the system to store assessment history temporarily, or should it generate the PDF and discard the data immediately?
• Do you already have a branded report template, or would the PDF design need to be created from scratch?
• Do you have specific health thresholds and recommendation rules already documented, or should these be developed as part of the project?
• Will recommendations be fully rule-based (e.g., high falls risk = mobility aid guidance), or would you like AI-assisted narrative summaries included?
• Should the generated PDF be emailed automatically to the client and/or manager?
• Do you require electronic signatures from the assessor or client?
• Will HubSpot integration be required in Phase 1, or only later once HubSpot is launched?
• Are there any compliance requirements we should consider (GDPR, NHS-related processes, data retention policies, etc.)?
These answers will help determine whether a simple reporting tool is sufficient or whether we should design it with future HubSpot and operational workflows in mind.
