The problem is water ingress via the rear valley gutter of the chimney stack and also underneath the main gutter where the water is getting into the space between the top of the chimney stack and underneath the gutter, causing damp to the inside of the wall and to the timbers inside the roof space. If you study the pictures you'll see the water marks on the outside wall from the leaking gutter.
Erect scaffolding to comply with Health and Safety regulations. Remove existing lead flashing to all sides and back of the chimney stack and install 450mm C5 lead flashing tucked and wedged into brickwork.
Remove 5m of gutter and dispose from site. Check fascia boards for rot. Check render for soundness. Locate and repair any gaps found on top surface of chimney breast underneath gutter. Create and install 300mm C4 lead flashing apron over wall at eave level to run underneath roof tiles, over the fascia board and covering top of chimney breast. Point in lead works using 3/1 sand and cement mix with added water proofer.
Install 4 inch UPVC gutter, fixing with clips and screws. Watertest all gutters for correct fall and clean where needed. Replace wet timbers inside roof space. One coat of white masonry paint to outside rendered wall. Clear all rubbish and leave in a sound and watertight condition.
