JavaScript/PHP or other solution to track users through external site and back again

General information for the business: We recruit survey respondents
Description of requirements/functionality: I need a way to track people who fill in a basic form or short survey on our website and then get directed to an external survey, and then back to us. Each person needs to be assigned a unique User ID. They will be directed back to one of 3 pages on our website depending on how they answer the survey on the external site:
http://mywebsite.com/complete?uid=xxxxxx for respondents who have completed the survey
http://mywebsite.com/fullquota?uid=xxxxxx for respondents that we no longer need
http://mywebsite.com/screenout?uid=xxxxxx for respondents who don't qualify for the survey
We can ask the survey host to install code at their end, I just need to know what code and where they should place it.
Hopefully this makes sense! I'm open to different suggestions - not sure if the above could be achieved through Google Analytics, for example?
I'd ideally like this to be done in the next week.
