DFP mobile web interstitial template (not app)

We are looking someone to develop a creative template for mobile web (not app) interstitials (overlay pop-up) to be used with Google's Doubleclick for Publishers.
Description of requirements/features: Requirements:
- http://dfp.ingraye.com/creative/interstitial/ - you may modify this template so that it works on mobile web (renders ad to full page)
- possible HTML, javascript, jQuery, CSS but any other solution is welcome
- template is for mobile web, not app
- work with image uploaded to DFP as well as third party tags and DFA tags
- X for exit displayed at top right, ADVERTISEMENT displayed at the top (see 2nd example)
- template should work with different sizes of creatives
- contain cachebuster/click macros or placeholders for them
- basic documentation as to how to implement
