The project database needs to be able to store information of clients: name, age, address, diagnosis/condition. With this clients will choose an activity to be supported or information to be provided. (See attached referral and assessment forms for more fields and info on the two types of service we will provide). clients can be new, existing or re-referrals to the project. Once a client is archived although this maybe because they have disengaged/ill health or no more support is provided.
Every 1/4 I have to complete a report on referrals received for the 1/4 and for the annual referrals: location, age, gender, types of referral, clients support and their activities, archived clients and who referred.
The cost of this database is not an issue although quick response is, as this is urgent and the first to give a rough estimate will more likely get a response first.
