Description
Database management system (DBMS): Microsoft Access
Description of requirements/functionality: I am a personal trainer and massage therapist and have a mixture of mobile and private clinic based clients. To ensure I have a clear and professional record of all the interactions, training sessions and treatments administered I am looking for a database.
I use a similar MS Access application in another capacity that I can share screenshots with the successful proposer as this would be the sort of thing I'm looking for.
Below are some of the functionality requirements:
1. Have the ability to work on multiple devices (iMac desktop, Mac laptop and iPad)
2. Be added to my Microsoft Sharepoint
3. Capture contact, medical and NOK details
4. Health screening results (Blood pressure, press ups, sit ups, step test, body fat %, weight, height, max heart rate and zones)
4. Track client progress (Text input and Chart)
5. Be able to add notes
6. Be able to record SOAP notes
Extra notes:

Jay, as a professional I'm sure you know that gaining a skill can be an expensive business and no professional can earn a living without taking account of the time and expense acquiring those skills and paying for the equipment required to do the work.
Please think again about the budget. No professional can do this work at well below the minimum wage. I wouldn't employ a personal trainer charging below the minimum wage and you should think how professional/skilled a person can be at £5.60 an hour.Jay M.16 Apr 2017Thank you for your message. Whilst I agree it's expensive to acquire skills and obtain the equipment in which to use those skills, there are also a minority of people that exploit others lack of knowledge by proposing/charging astronomical amounts - which is just as wrong, if not even worse as those working for national minimum wage or below.
Fortunately, I am working with someone on this project and you'll be pleased to know are working well above national minimum wage as dictated by their skill level and qualifications. I think people on here need to think before they become a keyboard warrior and jump in with their opinions as they are often inaccurate as they are unable to see proposals coming in.
