There is elderly patient cohort who are medically stable but there is no place to send because of either mobility or social issues. There are no rehabilition beds, also no care home beds available.
We are a team of one medical doctor, one physiotherapist, one occupational therapist and will hire few Physio assistants & Healthcare assistants; the copyright idea is to create a private discharge team by giving relevant services to see patients with relevant assessments and expedite and facilitate the hospital discharges. This will be a business proposal and possibly a tender to submit. There can also be an add on business project, To develop and create "transit beds" in community.

Statistics data would be needed with potential possible options for; what, how much and how the services will be provided and can support the early hospital discharges and facilitate the long stay inpatients.
