Design a logo with multiple revisions

What the Freelancer needs to start the work

For my design process I'll require a simple questionnaire to be filled out.

1. Provide the name of the logo as it should be used in design.
2. Provide tagline or leave empty.
3. Shortly describe website (provide link if online )/business/product.
4. What is your prefered colour scheme ?
5. Are there any elements that shouldn't be included into the logo.
6. Provide images/links to the logos that appeal to you.
7. If you have any additional comments please include them in this point.
