I can design simple static to complex, fully dynamic pdf forms for you to email, print or put on your web site. For this gig, I will design from scratch (not just simply convert your form unless you wish to do so) a simple form in Adobe LiveCycle or Adobe Acrobat which can include your logo and up to 20 fields. For every additional set of up to 20 fields, please add an extra gig. Fields can include text fields, check-boxes, radio buttons, text boxes, buttons (including email submit button).
