Spanish translation to English

I have approx 150 completed market research questionnaires in Spanish that need translating into English on an excel spreadsheet. Each questionnaire is A4 size double sided with 2/3 of the questions open ended. I am looking for a price per questionnaire not per word as some questionnaires have lots of words and some not many at all - so an average price would be best. I will attach a sample questionnaire - answers to be translated are the ones in red.
