English To Spanish Translation

I have a standard two page word document which is called an enrolment form. It has various fields for a client to input on and a set of terms and conditions. I need someone to translate this document from the current English to Spanish. It needs to be accurate and have no errors. The document is approx 1000 words. I will not work with people who plan on using google translate or other online freebie software. This need to be completed by someone who is able to offer a professional English to Spanish translation that will be used by Spanish clients without queries. Subject to this working I will require other documents translated. I want to get this job done in a quick turnaround.
