Translation into MS WORD medical forms

I require several medical forms translated from English to Urdu. The forms MUST also match the layout and format supplied so whoever undertakes this must be proficient in MSWORD and general re-formatting

The prepared documents will the be proof checked bye a UK based employee who is fully proficient in Urdu so there maybe amendments required.

Once the freelancer has been selected they must respond within an hour they have accepted and the work MUST be delivered by no later than Monday 8th January by 1200 UK time. Forms are attached 3 in MS WORD and one as a PDF which will be sent separately later which will be required to be duplicated ( no conversion tools to be used) in MSWORD.

We are seeing this as an hourly rate NOT fixed price so we require an estimate of time and NOT after the job has been awarded.
