Documents Transfer from PDF to Google Docs

I have a document that is a PDF Sample that I need created in Google Docs.

It is a 'Moving and Handling Risk Assessment Form' for our elderly clients. Our Manager will print this new blank form each time she does the risk assessment for an elderly client.

See the screenshots attached to give you an idea. I will create a blank Google Doc and share it with you so I will be the owner and you will be the Editor (until completion).

It is quite long, about 15 pages. I only have it in PDF and the file Size is 15 mb which is too big to put on PeoplePerHour.

But I can explain further details on email. It's a very simple form - just a long one.
