Convert Excel database into Word format.

I posted this yesterday but obviously didn't make my specifications clear enough, as from the many proposals I received no-one understood what I wanted, so I'll try again.

The author of a book I am publishing has produced an index in Excel format, and I need it converted into a word document or similar, suitable for inserting into the book in printed form. As well as main text it has Main Headings, Sub Headings, etc.
I'm attaching a copy of the index in its current state, and can also supply the full text of the book for reference in pdf format if needed.

I've attached a sample form an earlier book of how the finished index should look.

And to clarify even further, I need a result in a plain text file that looks something like this:

Alelaide, Queen, 27
Alexander II, 34, 38
BALLETS
Giselle, 67
Sleeping Beauty, 129
