Excel quality document

I need a short Excel quality document, a straight forward heading and subheading with a tick box yes/no column added, with company logo added. I also have more regular work like this as it is overflow that I do not have the time for. The one caveat is it needs to be complete in 4 hours. I have more projects like this if someone can provide the right prices. I have attached a rough document, this just needs to look like a quality document.
