Copywriting a policy

I have a flexible working policy we are not happy with the below point, the Policy gives the impression that once a Flexible Working Policy agreement is in place it’s for life. The words need to be changed to say it is not permanent and that it doesn’t affect their employment rights, could you take a look at this for me please?

7.0 Contractual rights
Any change agreed will be a permanent change to the employee’s terms and conditions of employment, unless a trial period is agreed.
Employees do not have the right to revert to their original or previous terms and conditions after a request has been agreed and implemented.

I need to email all our Academics and ask them to complete the flexible working form for the next academic year if they wish to apply for flexible working, and I need to direct them to a link where the revised flexible working policy lives- please could I get wording for this email?

Also I am not sure what to do with the colleagues who are expiring their flexible working policy this year, do I contact them or their line managers separately asking them to re-apply or shall I put it in the above email? If so can I have warding for that as well please.
